Where to File
HOW AND WHERE DO IF FILE?
As soon as possible after your accident occurs, you must put your supervisor
on notice that an accident has happened and must be reported. If possible,
report the incident further to your supervisors’ boss, and to the human
resources department of your employer. Do not turn down offers for
an ambulance or immediate referral to a hospital or doctor in the wake of
your injury. Do not try to “tough it out”. The sooner that you
seek medical treatment, the sooner that your claim will be taken seriously,
and your actual physical recovery will greatly benefit from such quick treatment.
Do demand to see and complete a blue “Claim for Benefits” form, which your
employer does have, regardless of what they may tell you. If you run
into difficulty with your employer on any of these issues, contact the Bose

As previously noted, your blue “Claim for Benefits” form must be completed
and signed by you, and forwarded directly to the Virginia Workers’ Compensation
Commission at 1000 DMV Drive, Richmond, VA 23220. Do not give this
form to your employer and assume that they will file it. Do it yourself,
or let our firm help you. Be sure to specify that you are requesting
specific wage loss and medical benefits on the claim form. This form
may be amended following the initial submission, but your claim may be limited
to those benefits requested in the claim form, so it is important to be
